Life Insurance

Basic Life and AD&D Insurance

WVURC provides Basic Life and Accidental Death and Dismemberment (AD&D) through Principal at no cost to benefits-eligible employees.
  • The Basic Life benefit is equivalent to one year of your annual salary (rounded to the next higher $1,000) with a maximum of $165,000.
  • Coverage reduces 35% upon turning age 70, with an additional 20% reduction at age 75.

Voluntary Life

In addition to the provided basic life insurance, you may also purchase additional life insurance coverage through Principal for yourself, your spouse, and your legally dependent children. Your new hire period is the only time you may receive the guaranteed issue (GI) amount without providing health information. To request greater than the GI amount, you must submit an evidence of insurability (EOI) to Principal.  If you decline voluntary life during your new hire period, your GI amount will be $20,000 during Open Enrollment.

Employee Voluntary Life

  • The benefit amount is selected in increments of $10,000 with a minimum amount of $10,000 and a maximum amount of $300,000.
  • The guaranteed issue amount if you're under age 70 is $100,000. If you're 70 or older, it's $10,000.
  • Coverage reduces 35% upon turning age 65, with an additional 15% reduction at age 70.
  • You can't enroll in spouse or child voluntary life without being enrolled in employee voluntary life.

Spouse Voluntary Life

  • The benefit amount is selected in increments of $5,000 with a minimum amount of $5,000 and a maximum amount of $150,000. The amount of coverage may not exceed 100% of the employee amount.
  • The guaranteed issue amount if your spouse is under age 70 is $30,000. If your spouse is 70 or older, it's $10,000.
  • Coverage reduces 35% upon turning age 65, with an additional 15% reduction at age 70.

Child Voluntary Life

  • The benefit amount for child(ren) up to age 26 is $10,000.
  • Dependents under 14 days old receive a $1,000 benefit. 

Designation of Beneficiary(ies)

In the event of your death, your life insurance policy benefits would go to the person or people you have designated as your beneficiary(ies) in UKG. To enter or update your beneficiary information, follow the below instructions:

   1. Log in to UKG (wvurc.ultipro.com) with your WVU credentials.

   2. Click on the ‘Myself’ tab on the upper left side of your screen.

   3. Scroll down and click on the ‘Life Events’ tab.

   4. Click on ‘I want to change my beneficiary’.

   5. Enter the current date for “when did this happen?” under the ‘About this Life Event’ tab and click         ‘next’.

   6. At the ‘Verify Beneficiary and Dependent Information’ tab:

            a. If the person(s) you’d like to designate is/are already listed:

                    i. Click on their name.

                    ii. Click on ‘edit’.

                    iii. Make sure all information is entered and correct, including legal first and last                                         names, SSN, and date of birth.

                    iv. Under ‘Designation’, make sure ‘beneficiary’ is selected.

                    v. Click the blue ‘save’ button in the top right corner.

            b. If the person(s) you’d like to designate is/are NOT listed:

                    i. Click on the blue ‘add’ button at the top right corner.

                    ii. Enter all information and click ‘save’.

                    iii. Follow all steps listed above in 6a.

   7. Once your beneficiary(ies) are designated, click ‘next’ and follow the prompts through to the                ‘Confirm Your Elections or Changes’ tab.

   8. If everything is correct, click the blue ‘submit’ button at the top right to finish.

You may change your beneficiary(ies) at any point in time.
